A review of your long-term condition is important to improve health and outcomes.  You will be invited to attend the surgery for an annual review of your long-term condition every year around your birthday month.

Regular review of your condition and associated medications is necessary to reduce the risk of complications from your condition and ensure you are receiving the most appropriate care.

You will be invited in any twelve month period.  Failure to respond may result in a delay in obtaining your repeat medication and/or reduced supplies
